10. What Is my financial responsibility for services rendered? (top of page)

Your financial responsibility depends on a variety of factors, explained below.

We ask that you pay any co-pays necessary at the time of check in. Our office calculates expected patient liability based on information that is provided by your insurance carrier. Please note that the liability amount is an estimate and as such subject to change once your carrier adjudicates the claim. Our office makes every effort to verify your benefits in advance of your visit. Please advise our office of any insurance changes in advance of your visit in order to minimize the risk of financial exposure for all parties. It is your responsibility to insure that our office is informed prior to services rendered of any changes. Likewise, your assistance in timely response from your insurance carrier is requested in the event additional information is requested.

11. What if my child needs to see the physician? (top of page)

The practice accepts patients that are at least 15 years old. A parent or legal guardian must accompany patients who are minors on all office visits. This accompanying adult is responsible for payment of the account, according to the policy outlined on the previous pages.
